A GP Chronic Condition Management Plan (GPCCMP) is a tool used by GPs to assist a patient in developing a structured approach to managing their chronic condition. This is done by stating the patient’s health needs in managing their chronic condition, developing health and lifestyle goals and identifying actions needed to be taken by the patient and what other services the patient requires to manage their condition.
A review appointment would be required to asses a patient’s progress and what adjustments need to be made for ongoing management. This review appointment would be used to determine if a patient was able to reach the goals set in their original GPCCMP appointment, what worked and what didn’t and what can be done moving forward for the best possible care.
The is no Medicare approved list of eligible conditions for GPCCMPs, a Patient is eligible for a GPCCMP when they have one or more medical conditions that have been, or is expected to last at least 6 months, or is terminal. A GP can only do a GPCCMP if they:
A GPCCMP would also allocate up to 5 rebateable sessions with an allied health provider per calendar year to assist with ongoing management of a chronic condition, unused sessions do not rollover onto the next calendar year.
A Patient who is registered with MyMedicare must complete their GPCCMP with the provider they are registered with. If you wish to have a care plan done by a GP other than your registered provider you must either:
An appointment will typically last 45 minutes, beginning with a 30-minute session with the nurse, followed by 15 minutes with a GP, this applies for both a new GPCCMP and a review. A GPCCMP will be either bulk billed to Medicare or charged at the 100% rebate rate for OSHC holders.
A patient is eligible to have a GPCCMP Review 3 months and 1 day after the last new plan or review was completed. However, exceptions can apply under exceptional circumstances, such as the development of a new chronic condition, a change in severity of the condition, etc.
A Health Assessment is a comprehensive examination of a patient’s health, psychological and social wellbeing. This includes an evaluation on whether preventative health care and education should be offered to a patient.
